Prior to your appointment, you will be asked to complete intake paperwork and provide relevant background information.

Depending on the reason for referral, this may include:

  • Medical and psychiatric history

  • Current symptoms and concerns

  • Developmental history

  • Medication history

  • School or work-related concerns

  • Prior treatment records

Providing complete information in advance helps ensure your appointment time is used efficiently and thoughtfully.

Your first appointment will involve a comprehensive psychiatric evaluation.

During this visit, we will discuss:

  • Presenting concerns

  • Symptom history

  • Medical and psychiatric background

  • Developmental history (when applicable)

  • Functional impact on school, work, relationships, or daily life

  • Treatment goals

The purpose of this visit is to gain a clear understanding of your situation and begin developing an individualized treatment plan.


Treatment planning

Following evaluation, recommendations may include:

  • Medication management

  • Behavioral or lifestyle strategies

  • Therapy referral or coordination

  • School or work accommodations

  • Additional diagnostic assessment when appropriate

Treatment recommendations are tailored to the individual and discussed collaboratively.


Patients receiving ongoing care will participate in follow-up appointments to monitor progress, assess response to treatment, and make adjustments as needed.

Frequency of follow-up depends on the nature of treatment and clinical needs.
